Medical/Prescription
Drug Benefit
A Preferred Provider Option (PPO) plan provides comprehensive medical and health services, typically subject to a co-payment. Prescription drugs are covered for both retail pharmacies
and mail order. A Consumer-Driven Healthcare
Program,
which includes a company subsidized health reimbursement account,
is also available.
For the medical and prescription coverage, the Company pays a varying percentage towards the cost of your coverage based upon the plan selected, and the tier of coverage.
Dental
Benefit
The
plan pays benefits for the following services: Preventative and
Diagnostic care, Restorative (Including Major Restorative),
Oral Surgery, and Orthodontia Care.
Company

Life
Insurance Benefit
The
life insurance amount selected is payable to your beneficiary upon
your death.
1x salary up to $50,000 Group Life (Fully paid by Company)
Supplementary
Group Life is available for an amount of up to two-and-a-half
times your annual base salary (Employee pays 100% of cost)
Short-Term
Disability Benefit
This
benefit provides for the continuation of salary (based on length
of service) to you if you become disabled due to a non-job-related
sickness or injury. You must be certified as disabled by a licensed
physician and subject to carrier approval.
Fully

Long-Term
Disability Benefit
This benefit provides for an income continuation benefit if you become
totally and permanently disabled beyond 180 consecutive days due to
a job- or non-job-related disability to full time employees. You must be certified as disabled
by a licensed physician and subject to carrier approval.
Company

Business
Travel Benefit
If an accident occurs while you are traveling on Company business,
the plan provides benefits for accidental death, dismemberment, or
total disability. Coverage is two times your base salary (up to
$250,000).
