Thank you for your interest in the Utica National School Safety Excellence Award Program.

Based on feedback from our policyholders, we have developed this secure document upload site which can make it easier for you to provide your school’s information.

Your school should have received an email with the electronic spreadsheet and instructions for completion.

Completed forms and documentation, if applicable, are due by February 1, 2025
NOTE: Support information is only required if you did not submit supporting documentation last year or if substantial changes have been made to last year's documentation.

To upload your files, please combine them into a single zipped folder, complete the required contact fields below, and click Choose Files to add the zipped folder. Once complete, click Submit. You should then receive an email from our team confirming receipt of your submission.
